WHAT IS A COMPLETE BREAKFAST OR LUNCH MEAL?

Well balanced and nutritional meals are planned daily.  Meal pricing offers students a complete meal with a wide variety of choices to meet the needs of the age groups served.

Meals consist of components or units.  Entrées consist of 2 components (In general a protein or meat and grain combination).  A colorful selection of vegetables and fruits are considered 1 component each, with 2 offered with lunch and 1 with breakfast. Milk is 1 component.  The total number of components offered at lunch is 5.  A Complete Lunch includes a choice of entrees, a colorful selection of vegetables, fruits, juices, bread or grains, and milk.

*Students must choose at least 3 of the 5 components offered with at least one serving of a fruit or vegetable to receive the meal pricing.

*If a student takes anything less than 3 components and a fruit or vegetable is not chosen ala carte prices will be charged and this will most likely result in a higher cost to the student.  A Complete Breakfast includes a hot or cold breakfast entrée, fruit or juice, and milk. Students must choose a total of 3 of the 4 components offered to receive meal pricing.

ADDITIONAL INFORMATION REGARDING FOOD/BEVERAGES

All food on campus is selected using the highest standards and meet the USDA requirements for school meals.

·         Meals are planned to meet the new standards that include each week an offering of dark green, orange/red, starchy, legumes, and other vegetables.

·         Larger portions of fruits and vegetables will be offered to students based on the grade level served.

·         All milk is non-fat and if offered in both plain and flavors.

Acceptable snack items that are nutritionally approved are available at additional charges.

UPDATE RECORDS THROUGHOUT THE YEAR

It is imperative that household information is updated AT YOUR CHILD/REN’S SCHOOL whenever information changes.  These changes include, but are not limited to, the following: change of name, change of address, change of household members, and change of telephone numbers (including emergency contact information).  This information must be updated regularly at each child’s school so that adequate record-keeping can be maintained.  Information that is NOT updated can cause delays in the processing of Free and Reduced Meals Applications.  Also, without correct information, important mailings from the district will not reach you.

FOOD ALLERGIES

Please refer to the district’s policy and guidelines STUDENT’S WITH FOOD ALLERGIES if your child has a severe food allergy.  These are available on the district’s website or you can receive a copy from your school.

Please understand that the food service department tries to accommodate special needs, but medical documentation from your physician’s office is required and must be updated annually for any medically required substitutions. The routine medical information on file at the school is not shared with the Nutrition Service Department unless it is specifically ordered to be shared.  Elementary schools provide foods that are nut-free.  Secondary schools, however, do not.  If your child has a severe food allergy, please make all appropriate staff aware of the issue.

PARENT PORTAL

If you would like to view your child’s meal account activity, grades, absences, fees, etc., you need to go to the link called Parent Portal which can be found on the homepage of the Mentor Schools website.  To set up your account, you will be given one extremely long Campus Portal Activation Key, per student.  You will only need this key to set up your account.  After the initial set-up, you will no longer need to enter in that key.  For more information on this, please contact the Information Technology (IT) Department.

CARDCREDIT (EZ-Pay)

If you would like to make online payments for meals or for school/activity fees, you need to go to the EZ-Pay site.  The link to this site is called CardCredit Meal and Fee Payment and can be found on the left hand side of the homepage of the Mentor Schools website.

Here is the information that you will need in order to set up an EZ-Pay account:

    Click on Log In.
On the right hand side there is an arrow that says Register to use SPS EZpay.  Click on that arrow.
You should now be on a welcome page.  After you read the information, click on Next.
Enter your information and click on Next.
Enter your child’s student id number, your child’s last name, and click on Add Student to List.
    Click Finish.

PLEASE MAKE SURE TO KEEP YOUR CONFIRMATION NUMBER FOR ANY DEPOSIT MADE UNTIL THE ACCOUNT ACTUALLY SHOWS THE POSTING!  It can take a few days for this to happen.  If the cafeteria managers are aware that the deposit has been made, your child will still receive meals.

ACCOUNTS WITH NEGATIVE BALANCES

As mentioned before, it may take a day or two for an online deposit to actually be posted to your child’s account.  EZ-Pay sends daily payments reports to the Food Service Department.  These are forwarded to each cafeteria.  If a child has a negative balance, s/he will be able to “charge” up to three (3) meals.  If the account has not been reconciled after the third meal is provided, an alternate lunch will be given to the student.  Also, snack and ala carte items are not permitted to be “charged” when there is a negative balance.

ACCOUNT BLOCKS

If you find that your child is purchasing food items or beverages in excess or without your consent, a block can be put onto your child’s account.  The block can be implemented in different ways: daily limit restriction, ala carte block, or single purchase restriction.  The block will remain on your child’s account for the remainder of the school year unless you call to have it removed.  The blocks do NOT carry over into the following school year.  If you would like more information regarding blocks, call the Nutrition Service Department at 974-5227.

PIN NUMBERS AND PIN SCANNERS

Five digit pin numbers are assigned to each student.  The pin number will remain exclusive for that student and will follow him/her through his/her school years at Mentor, even if you leave the district and then return.  It is extremely important that your child memorize this pin.  Memorization helps to facilitate decreased time in the lunch line.  Otherwise, precious time is used to do a search for these numbers.  The keypad below is what we use in our schools.

* New this year is the addition of PIN Scanners at all elementary schools.  This greatly assists our students by simply holding their pin card under the scanner.  It immediately recognizes the students PIN number and their account appears on the terminal screen for the nutrition staff member to access.  This is a tremendous advantage especially to our younger students while they are learning their numbers.

FREE & REDUCED MEALS APPLICATIONS

First and foremost, all information shared on your Free and Reduced Meals Application is confidential.  If you think your family might be eligible to receive free or reduced meals, you are encouraged to fill out a Free & Reduced Meals Application.  The applications are mailed to all Mentor students’ homes during the latter part of July.  They can also be obtained from the Food Service site by clicking on the Food Service tab on the homepage of the Mentor Schools website, as well as in each school building.  An application can be submitted at any time during the school year.

In order for the application to be processed, please make sure you closely follow the directions provided on the application.  Here are a few extra tips:

*Make sure ALL household members are listed in Section I.

*Make sure you list each child’s school.

*If you have a SNAP or OWF case #, include it next to each

child’s school name in the boxes provided.

*Make sure the income and the frequency for each household member is included.  (Note: bi-weekly means every other week, bi-monthly means twice a month)

*Make sure you mark either Yes or No as to whether or not you agree to have your application information shared with Student Services regarding school fee waivers, and sign and date this section, as well.  (More information can be found in the School Fee Waivers section.)

*Make sure you SIGN and PRINT your name; DATE the application, include your ADDRESS, PHONE NUMBER, and SOCIAL SECURITY NUMBER.  You do not need to include your social security number if you have listed a SNAP or OWF case number.  IF YOU DO NOT HAVE A SNAP OR OWF CASE NUMBER, YOU MUST INCLUDE YOUR SOCIAL SECURITY.  YOUR SOCIAL SECURITY NUMBER, IF YOU HAVE ONE, IS REQUIRED BY LAW, AND YOUR APPLICATION CANNOT BE PROCESSED WITHOUT IT!!

Children’s eligibility for free or reduced meals is confidential.   All student eligibility is programmed into our computer point of sale system.  There is no identification that would “show” your child’s eligibility.

 INSTRUCTIONAL FEE WAIVERS

School fee waivers will not be considered unless you are approved for Free or Reduced Meals.  Parents/guardians must give written permission to the Nutrition Service Department in order to release confidential information for INSTRUCTION FEE WAIVERS.  Section 5 of the FREE AND REDUCED MEAL APPLICATION grants this permission.  Please complete this section for consideration.
