Dear Parent or Guardian,
According to the Ohio Department of Health/ Ohio Revised Code 3313.671, all students in the following grades must have completed the required immunizations.
All students entering 12th grade are required to have the Meningitis vaccine given on or after the age of 16 years of age.
All students entering 7th grade are required to have TDaP and Meningitis vaccines
All students entering Kindergarten must show complete immunization for DTaP, Polio, Hepatitis B, MMR and Varicella. Your child will be due for many of these immunizations after the age of 4 years.
Please make arrangements with your child’s health care provider or the Lake County Health District 440-350-2554 to receive the required immunizations.
Written proof of these vaccines must be turned in to the school office by September 13, 2019. Failure to do so will result in your child not being able to attend school and all school-related activities until received.
There are several choices that may be made to resolve this issue:
Have the child immunized and present verification (vaccination record and/ or immunization confirmation form signed by child’s healthcare provider).
Inform your school nurse of the date your appointment is scheduled to receive the vaccine and where. The nurse will need verification of the vaccine following the scheduled appointment.
Sign a waiver stating that you do not wish to have your child immunized due to religious or philosophical grounds.
If there are medical reasons as to why your child has not received the vaccine, a medical waiver should be completed by your child’s physician.
Again, this needs to be completed and updated within our records by September 13, 2019. If you have any questions, please contact your school clinic.
