Non-Public School: Transportation Request Forms
To ensure efficient school bus routing for the upcoming school year, a new Transportation Request Form must be submitted annually. Mentor Schools requests this of current non-public families by May 31st for the next academic school year to guarantee transportation for the 1st day of school. We will do our best to have your child routed for the first day of school if this form is received after July 15th.

Determine Eligibility by Thirty Minutes Direct Travel Time
Districts are not required to transport elementary or high school students to and from a chartered nonpublic or community school if the direct travel time exceeds 30 minutes. The time period is determined by driving a school bus from the public building to which the students would be assigned if attending the public school designated by the district of residence to the enrolled chartered nonpublic or community school. Initial eligibility timing for the 30 minutes is the responsibility of the public district (Ohio Dept. of Education and Workforce, Feb. 2025).
